Main Duties of Placement

The FY2 Hospital at Home placement at the WGH is primarily based in the Assessment and Rehabilitation Centre (ARC), which is a MOE MDT outpatient clinic. Patients attend once per week over a 6-week period and are assessed by medical staff, physiotherapy and occupational therapy teams. Duties involve clerking new patients and reviewing their progress over subsequent weeks. Common presentations include patients with reduced mobility and falls, cognitive decline and multiple comorbidities. Two days per week is spent in the community, working with various community services, general practice, MOE consultant home visits, and at hospital-based complex care sites (HBCC). On-calls involve working in the ARU at the WGH clerking patients and nights covering medical wards at the WGH.

Typical Work Pattern

Typical working week is Monday to Friday 9am to 5pm, with one in four weekends covering ARU 9am to 9:30pm.
