This post will offer experience in Community Medicine in the Hospital at Home Team along with duties in the Medicine for the Elderly service and some shifts in Acute and General Medicine in a supported environment. You can expect a minimum of 4 weeks time in the Hospital at Home Service.

Main duties:

• Participating in the work of the Hospital at Home REACT team including attending virtual ward rounds, visiting patients at home, clerking in new patients, devising management plans and working alongside the Consultants and Advanced Nurse Practitioners  and the REACT rehab team.

• Acute medicine on call shifts in the Medical Admissions Unit (MAU) and Emergency Medical Admissions (EMA)

• Clerking in new patients, organising investigations, and initiating appropriate management.   Participating in acute receiving ward rounds. Working as part of the medical emergency team.

• Day shifts on Medicine for the Elderly Wards (Frailty, Orthogeriatric Rehabilitation and Stroke)

• Duties will include daily ward rounds, continuing medical care for inpatients, discharge planning and communication of clinical update and management plans with patient and relatives.  You will work within a multidisciplinary team consisting of nursing staff, medical nurse practitioners, pharmacists, physiotherapist and occupational therapists.

• Evening shifts covering medical wards

• Hospital at Night and Hospital at Weekend shifts

Will work as part of a team providing out-of-hours care for acute admissions and existing inpatients in the medical wards

There is a multi-disciplinary medical unit educational meeting every Tuesday. There are also specialty educational meetings which will be aligned with your ward base.  St John’s Hospital has a well-equipped clinical skills lab.  Supportive clinical supervisors and middle grade training doctors will provide feedback on your clinical work and assist in completion of Supervised Learning Events and direct you towards additional learning opportunities.

You will gain experience in managing a wide range of medical conditions in a supportive medical department.  You will also gain insight into working as part of a large multidisciplinary team.
