Placement Community Geriatrics (Community)
Year F2

Main Duties of Placement

The FY2 Community placement at the WGH is primarily based in the Assessment and Rehabilitation Centre (ARC), which is a Medicine of the Elderly (MoE) multidisciplinary outpatient clinic.

Patients attend once per week over up to a 6-week period and are assessed by medical staff, nursing staff, physiotherapy and occupational therapy teams. Duties involve clerking new patients and reviewing their progress over subsequent weeks.

Common presentations include patients with reduced mobility and falls, cognitive decline and multiple comorbidities.

One day per week isspent in the community, working, with the IMPACT Team who support patients with chronic disease management. One day per week is spent supporting the Acute Frailty Ward Round in the Admissions Unit where experience is gained at the hospital/community interface.

Evening and weekend shifts are as part of an evening on-call team and are based within the MoE unit providing ward cover. Some shifts may be based at the front door assessing acute all age patients.

Typical Work Pattern

Basic day shift is 0900-1700, evening shifts are 1630-2115, long day and weekend shifts are 0845-2115 and night shifts are 2100-0930 (as part of the Hospital at Night Team).

All are full shift pattern.

Learning Opportunities

The placement offers a variety of settings and learning opportunities involving outpatient clinic, home visits and acute medicine.

Teaching

Hospital-based teaching includes weekly Grand Rounds and weekly FY2 teaching programme.

Unit MOE teaching includes weekly radiology meeting, weekly multidisciplinary teaching and weekly junior doctor tutorial programme.

The unit has a regular Morbidity and Mortality meeting and QI meeting which the FY2 attends. 

There are many teaching opportunities with a large number of medical students rotating through the department.
