Individual Placement Description
Placement Intensive Care Medicine
Year F2

Main Duties of Placement
  1. Working as part of a large multidisciplinary team, Foundation doctors will be expected to undertake daily reviews of critically ill patients in both HDU and ICU.
  2. Foundation doctors will be expected to assist in the assessment and admission of critically unwell patients to the ICU.
  3. Foundation doctors are expected to learn how to approach the initial assessment and resuscitation of the critically ill patient.

 

Typical Work Pattern
  1. There are 2 separate medical rota tiers in the Royal Infirmary Edinburgh ICU. Foundation doctors will work alongside core trainees from anaesthesia and ACCS, clinical development fellows and acute medicine specialty trainees. They will be supported by Specialty trainees from Intensive care medicine and anaesthesia training programmes and Senior visiting fellows who will work on a separate rota. The medical staffing in the critical care department is also supported by a separate advanced critical care practitioner rota (ACCP).  Each of the 3 rota tiers provide 24 hour cover throughout the working week.
  2. Foundation doctors will be allocated to a team of doctors and ACCPs working within one of the 4 critical care bases and will have access to senior support at all times. There will be consultant led ward rounds in each base twice a day and consultant presence within the department between 0800-0000 and on call between 0000 – 0800. There will be 2 senior registrars covering within the department 24 hours a day.
  3. The rota has a full shift pattern including nights and weekend working. 
Learning Opportunities

Foundation doctors are encouraged to participate fully in the activities of the department and over the course of their placement should become proficient in the assessment of the critically ill patient, the initiation of organ support and the use of invasive lines and monitoring. Foundation doctors will be given the opportunity to learn placement of arterial lines and if they complete the CVC mastery programme will have the opportunity to place central venous access devices.

We encourage attendance at foundation teaching and provide monthly protected departmental teaching (attendance at which is mandatory and included in working hours calculations). We provide a full induction process and a flexible working pattern to suit the leave requests of the entire department.

The department also provides access to weekly grand rounds, weekly journal club and weekly M and M meetings.

For those who are interested we have an active QI team with many opportunities to become involved in new and ongoing projects within the department.
