Internal Medicine Training in Scotland is delivered on a regional basis in the North, South-East, East and West of Scotland. Each region has a different range of hospitals and training experiences from busy urban district general hospitals, large regional specialty centres, renowned teaching hospitals, to remote and rural sites, all of which provide valuable and unique clinical experiences.
Although your clinical training is regionally based, the training programme directors in Scotland work together to provide a teaching programme for all internal medicine trainees, along with an intensive simulation programme for each of the years of internal medicine. You will meet with other trainees from all over Scotland at bootcamp in year 1 at the national simulation centre in Larbert, and also at our face to face events for quality improvement and national trainee conferences. The regular clinical teaching programme is delivered online with monthly afternoon sessions which are recorded and you will have access to the whole teaching programme which is designed on a three year rolling programme to cover the IMT syllabus. There are also national palliative medicine teaching sessions in addition to your attachment to palliative medicine teams during your clinical training.
Your study leave budget is supplemented in Scotland with additional separate funding for clinical simulation so this does not impact on your personal study leave choices. The simulation programme is highly valued by trainees and has been described as the best clinical teaching ever experienced by previous trainees. There is a three-day bootcamp in year 1, a clinical skills day in year 2 and a two-day registrar ready course in year 3. The clinical teaching programme is delivered over and above this budget with expert speakers from all over Scotland and trainee feedback of these sessions is excellent.
Research, audit, teaching and management opportunities
The Clyde hospitals for IMT include Golden Jubilee National Hospital (GJNH) in Clydebank, Inverclyde Royal Hospital (IRH) in Greenock and Royal Alexandra Hospital (RAH) in Paisley.
The GJNH is home to regional and national heart and lung services and for this rotation it offers a highly sought after cardiology post for 6 months of IMT3. This gives a unique and immersive opportunity to work in a cardiology unit which is part of the West of Scotland Optimal Reperfusion Service. It has the fastest ‘door to balloon’ time in the UK for treating patients having a heart attack caused by a blocked artery.
There is an enthusiastic team of consultants who provide educational supervision and it is an excellent training experience.
The IRH is a district general hospital which serves a wide geographical region including Inverclyde, Largs, Isle of Bute and the Cowal Peninsula. There is an amazing learning opportunity for general medicine experience. The team of consultants are highly committed to providing excellence in GIM training and educational supervision and encourage each resident to achieve their goals and flourish. As a department they regularly see colleagues returning to take on more senior roles including consultant posts and choosing to be part of this enthusiastic and motivated team.
There are fantastic Quality Improvement (QI) opportunities and a local annual QI Conference each Summer, where residents showcase their work with presentations and posters.
There is an established Chief Resident programme which is an ideal opportunity for developing leadership skills and being directly involved with the management team. There is also a Morbidity & Mortality (M&M) lead and a QI lead appointed each year.
The RAH is the main hospital in Paisley and also serves the population in the surrounding areas as far as Oban and Argyll. It is a very busy medical department and provides a wealth of clinical experience in general medicine. It consistently provides sought after training experience including in the Critical Care block of the IMT curriculum. It provides this service for the IRH and RAH, with excellent feedback received each year. The team of consultants are highly motivated to provide teaching, support and supervision and ensure that each resident reaches their full potential.
There is ongoing opportunity for QI involvement and there is a Chief Resident programme which affords the opportunity for developing leadership skills and linking in with management.
All three training sites provide regular teaching programmes weekly (at least), and these are invaluable opportunities for residents to take part.
